Flint Water Crisis
Core issue: Flint switched from Detroit's treated Lake Heron water to Flint River water, which was more corrosive. The corrosive water leeched lead from pipes, causing widespread lead poisoning.
Normally cities use things like "corrosion inhibitors" to help this problem. These were not used.
